Summary: Under the supervision of the CMS manager, assesses clients with chronic medical conditions, identifies their health care needs, and develops and implements plans of care to optimize healthy outcomes. Receives referrals and makes initia contact with family. Explains purpose and scope of CMS (Children’s Medical Services) program. Schedules intake appointment and meets with family to assess medical and social information. Documents all client encounters and maintains accurate medical records. Provides care coordination services and education to clients and their families with links to community resources and services. Maintains accurate, detailed reports and client records. Adheres to HIPPA regulations. Prepares clients and their families for the transition to adult care.

Minimum Qualifications: Bachelor’s degree in Nursing from an accredited college or university AND current Georgia license to practice as a Registered Professional Nurse AND CPR certification OR Associate’s degree in nursing from an accredited college or university AND One year of PHN experience AND completion of a PHN course within one year of hire AND current Georgia Registered Professional Nurse license AND CPR certification.

Preferred Qualifications: Experience with case management, children and adolescents with chronic medical conditions and special healthcare needs.

Salary Information: $26 hourly (Part-time/No Benefits)
